CUSTOMER IDENTIFICATION PROGRAM

On September 11, 2001 our lives changed forever when the United States was attacked.  In an effort to protect you and our country, President Bush signed the USA Patriot Act into law on October 26, 2001.

In accordance with Section 326 of the USA Patriot Act all financial institutions are required to do the following:

• Verify the identity of any person seeking to open an account – including deposit accounts, loans, trust accounts, investments and safe deposit boxes

• Maintain records of the information used to verify the person’s identity

For all persons requesting to open any type of new account relationship with First State Bank of Beardstown or First State Bank Rushville, we will require current photo identification and proof of your social security number.  We will also verify other information given by the customer.

If original documentation was not obtained with the opening of a prior account, it may be requested upon opening any new account.

In all cases, protection of our customer’s identity and confidentiality is First State Bank’s pledge to you.
